2. BIRTH CERTIFICATE with raised seal (must be issed by town or state where born, not hospital or “Record of Birth”, or previous passport or Naturalization Papers. Long form mandatory for children under 18 yrs. Office may no longer accept birth certificates issued by this office; must be issued by town where birth occured or State Health Dept.
3. TWO PASSPORT PICTURES - 2” X 2” identical, official passport photos only.

5. DRIVER’S LICENSE or other form of PICTURE identification (work badges or CT State ID from Motor Vehicle Dept. are not acceptable)
FOR CHILDREN 16 YEARS OF AGE OR OLDER –Child must sign his/her own passport application, but must be accompanied by parent and parent’s driver’s license. ALL PERSONS, INCLUDING CHILDREN OF ALL AGES, MUST BE PRESENT WHEN THE PASSPORT APPLICATIONS ARE SUBMITTED IN OUR OFFICE.
FOR CHILDREN UNDER 16 YEARS OF AGE – both parents must sign application in front of Agent and present driver’s license or valid identification. If only one parent is available, call for instructions. ALL PERSONS, INCLUDING CHILDREN OF ALL AGES, MUST BE PRESENT WHEN THE PASSPORT APPLICATIONS ARE SUBMITTED IN OUR OFFICE.
FOR CHILDREN 16 YEARS OF AGE OR OLDER WHO HAVE A DRIVER’S LICENSE – must sign their own application and present their driver’s license to Clerk. Do not have to be accompanied by adult. ALL PERSONS, INCLUDING CHILDREN OF ALL AGES, MUST BE PRESENT WHEN THE PASSPORT APPLICATIONS ARE SUBMITTED IN OUR OFFICE.
FOR CHILDREN 16 YEAR OF AGE OR OLDER WITHOUT DRIVER’S LICENSE – child and parent must both sign application in presence of Clerk and parent must present driver’s license. (Children 16-18) ALL PERSONS, INCLUDING CHILDREN OF ALL AGES, MUST BE PRESENT WHEN THE PASSPORT APPLICATIONS ARE SUBMITTED IN OUR OFFICE.
PASSPORTS ARE MAILED DIRECTLY TO APPLICANT WITHIN 4 to 6 WEEKS (or 3 weeks for expedited) FROM APPLICATION DATE, DEPENDING ON SEASON. ONCE THE APPLICATION IS MAILED, THERE IS NOTHING THAT THE TOWN CLERK CAN DO TO EXPEDITE THE APPLICATION.
